Premier Health is the Employee Health Provider for all SANE of Butler County employees.

Services are rendered upon a walk-in basis with no appointment necessary at your choice of 3 Premier Health clinic locations (Middletown, Dayton, Troy).

 

Their organization will handle all pre-employment, annual, injury medical assessments as needed. When employees seek treatment there is no upfront cost to the employee. Services are billed directly to the employer. 

 

All necessary documents are sent directly to the employer upon service visit completion.

• Influenza vaccination

• TB testing (PPD, QuantiFERON gold, xray)

• Hepatitis B titers, vaccine series

• MMR titers, vaccine series

• Injured worker medical assessments

• Pre-employment physical, urine drug screening

• Annual health requirements

• Other misc health services
